What to bring to your first visit?

We work hard to ensure that your child receives prompt medical care in a comfortable, child-friendly office setting. To help expedite the first appointment, please print out the "New Patient Forms" in the below section and have them filled out prior to your visit.

If you are transferring from another pediatric practice, please call our office and we will discuss the process to get your records and schedule a visit.

Patient Forms

We are also happy to offer you the option to download, print and complete our office forms to reduce wait times during your first appointment.

  • TSSAA Sports Physical History Form : Fill this out prior to your appointment if your child it planning on playing school sports. Please make sure to explain all "Yes" answers in the space provided in the bottom right.

  • ADHD Forms : If you are concerned about ADHD with your child, it will help us to have these forms filled out prior to your child's appointment. If this would be a new diagnosis, please fill out the initial parent form. Also, have one or more teachers fill out the initial teacher form. It is beneficial if this is done by the teacher who knows your child the best. Both the parent and teacher forms are required for us to get an accurate picture of your child. The follow-up forms are for patients who have already been diagnosed with ADHD that need follow-up or possibly medication adjustment.

What to Expect

On the day of your child's first visit to our office, we recommend arriving a few minutes early. This allows extra time for finishing up any registration forms and ensures you have plenty of time to get acquainted with our staff and office.
